By checking the box, and providing your mobile number, you (the End User) consent to receive SMS/MMS messages from Synergy Nutrition & Wellness LLC (the Business) for: appointment confirmations and reminders, scheduling/rescheduling updates, customer support communications, and promotional offers. Consent is not a condition of purchase or care.

SMS/MMS is not a secure, encrypted channel, and is not intended for emergencies or clinical communications. Do not send Protected Health Information (PHI) by SMS/MMS.

7. Privacy Policy: We respect your privacy. See our Privacy Policy for details. No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Other categories (outside of text messaging originator opt-in data and consent) may be used per the Privacy Policy.
